Pre-Registration

Online Registration

• Click the "Make An Appointment" link on the website.
• Enter the online register system.
• Choose first bespeak or subsequent visit of bespeak registration.
• Choose the department.
• Input your Individual Basic Data.
• Choose the doctor, date of seeing doctor and click "SEND".
• Print the registration page or write down the number of consulting a doctor.
• Please carry the registration slip to the clinic room. (First bespeak patients please check in at registration desk)
• If there are any problems when using the system, please contact via phone: 02-24282146 Ext. 101

Phone Registration

• Call the clinic service via phone: 02-24282146, ext 103 to 106.
• Consult the registration information, such as departments, doctors and available date.
• Tell the attendant your individual basic data, department, doctor, and the date/time you want to register.
• Ask to finish the registration and get the number of consulting a doctor.
• On appointment day, please go directly to the clinic room. (First bespeak patients please check in at registration desk)
• Operating Hours: 08:30-11:30, 13:30-16:00 (MON to FRI)18:00-19:30 (TUE & FRI night)08:30-11:30 (SAT)

FAX Registration

• Write down the individual basic data (name, ID No. birthday) clearly on the plain paper.
• Note the registration information, such as departments, doctors and the date/time you want to register on the paper.
• FAX the information via FAX: 02-24282149.
• After 5 to 10 minutes, please dial 02-24282146, ext 103 to 106 to confirm the FAX and finish the registration and get the number of consulting a doctor.
• On appointment day, please go directly to the clinic room. (First bespeak patients please check in at registration desk)
